Add or Update a Facility Name in Directory (Funeral Home, Hospital, Retirement or Nursing Home)

Instructions on reporting facility additions/name changes to directory via Outlaw

Any corrections to a facility name or to add new facilities needs to be reported to directory. The facility lookup feature during checkout on the sites and when adding a facility to location based fees comes from directory. Facilities on the help page also come from directory.

These changes need to be reported by opening a CT for 5500/5545. See example below.
